Evansville Care Campus
Our dedicated, friendly staff are eager to get to know you and provide personal attention with courtesy, respect and dignity. We hope to make your transition to our facility as comfortable as possible.

Evansville Care Center is a certified skilled nursing facility for Medicare, Medicaid and private pay.
​
Long-Term Care – For those who choose our facility as their new home and need to stay for an extended period of time.
Short-Term Care – For those who need short term rehab services after a surgery or illness that need some time to recover before returning home.
Hospice Care – We work with various local hospice agencies in a team approach to meet the holistic needs of our hospice residents.
The Facility
A bright cheerful place with a spacious dining room where residents gather for coffee and home cooked meals.
A cozy day room to visit with a friend or family member or to watch your favorite TV program or movie.
An elegant family room for gatherings, church services or a great place to relax and read a book.
The facility consists of 40 skilled nursing beds including 28 private rooms and six semi-private rooms. The rooms are large and offer the opportunity to add your own personal touch.

Services Available:

Nursing Services provided by licensed professionals and certified nursing assistants. IV therapy is available.

Medical Services provided by area physicians from Alexandria, Fergus Falls and Elbow Lake. The doctors from the facilities located in these towns make routine, in-house visits to stay current with residents’ health. We also have a clinic located in the lower level of our campus and they provide services from Prairie Ridge three times a week.

Rehabilitation Services such as Physical Therapy, Occupational Therapy and Speech Therapy are provided to help regain strength and allow residents to return to the highest possible level of function. The therapy provided is one on one allowing the residents to receive the right exercises for a quick recovery.

Outpatient Services are therapy services such as physical, occupational and speech provided at our campus to members of the community.

Restorative Nursing Services are provided by our nursing department under the direction of our therapists to maintain their level of functioning.

Dietary Services provide delicious and nutritious meals under the direction of a certified dietary manager. We also provide meal service to the community.

Activity Department offers a wide variety of individual and group programs to the residents, families and community members. Activities include live music, bowling, bingo, weekly church services, exercising and special events.

Specialty Services are offered at our campus and include a beautician weekly, podiatry that rounds on a regular bi-monthly, a wound nurse that rounds as needed and a dentist that comes twice a year.
